New patients need to arrive at least 15 minutes early for their appointment and bring their completed paperwork with them. First evaluation paperwork is located on our website and can be downloaded from the New Patient Forms page. Please note that the forms are very comprehensive and will take some time to fill out. Patients who arrive without their paperwork completed may be rescheduled.
Cancellations should be made at least 24 hours in advance to avoid a fee.
Our Ventura office hours are 9:00 AM to 2:00 PM on Monday, 9:00 AM to 5:00 PM Tuesday through Thursday. On Friday, office hours are 9:00 AM to 2:00 PM. Please call the office to schedule.
